I have a syntax highlighter package installed which has a settings-user file; if I want to add some settings to the package, must I do it on sublime-text user-settings file or the packages user-settings file ?

What kind of settings? Which package exactly?
Usually, packages provide their own menu item at “Preferences > Package Settings > “PackageName” > Settings - User” which will open the right file.

The package does have a Settings-User file, must I edit the Settings-User file for the package or can I do it all in the Sublime text Preferences-User File ?
If the package opens its own settings - user file then you must place settings in a file with that name. You may use subfolders however.
In other words, I have to use the packages settings-user preference file and make adjustment within that file, hrmm I was hoping I could make a giant sublime-text-users-settings file

Yes, you can place .sublime-settings file in sub-directories, as long as it has the same file name. Sublime Text will load them correctly, but I’m not sure how automatic saving (i.e. changes invoked by a plugin) respond to this - never tested it. The menu items will also not direct you to the file in the subdirectory because they are usually hard-coded to the User package.
