[Syntax] Entities' definition/reference not found

I am almost done with a new syntax, but somehow the Definitions/Reference shown when hovering over a scoped symbol are mostly missing or not correct (all files/directories are part of a project and indexing is on):

• it shows definitions for storage.type.my-syntax in indexed files of other languages with different file-extensions, e.g. c/c++
• it shows definitions also for keywords with scope e.g. keyword.conditional.my-syntax or keyword.control.my-syntax (also in other language files) which I would not expect

So the question is whether I am missing something fundamental…?

Scope-restricted symbol definitions

Right now, as far as I understand me (and please correct me if my understanding is incorrect), symbol navigation go roughly like this: you define a set of secopes, and any string in files with those scopes are registered as symbols in a single list. Then, when sublime encounter any string that matches an item in the list, it allows us to navigate to the place where the strig was captured.
