How do I change from the rounded tabs to the square tabs?
Right now it looks like this:

When I install a theme it only changes the colors, not the UI.

You’ll need to update your User settings file as well, instructions should be found in what ever theme you’ve installed.
Also note: There are ‘Color Schemes’ which only effect syntax highlighting and not ST UI.

when you switch theme you MUST restart st in order to render properly
