There are two things in current implementation of File / Open Recent that make them annoying (if not unusable).
First: both File / Open Recent and Project / Recent Projects should track deleted items and automatically remove them from the menu (at least on OS X). It’s how every other program works and shouldn’t be hard to add.
Second, more important: in my opinion, files that belong to a project (and opened from that project) should not be added to File / Open Recent. Having these files there just clutters up the list incredibly fast and pushes other files (that couldn’t easily be opened by re-opening a recent project) out.

I’d like to add:
You shouldn’t have to have a file (or window) open to see the list of most recently used files. Right now, if you don’t have some sort of something open, you can only see most recently opened folders. This is on Mac OS X. Unless I’m missing something obvious (which very well may be true).
