What is the relationship between the scope name and the file type? How do you specify a build system that works for *.md (markdown) files?

The relationship is defined by the syntax definition you are using to highlight/view the file. For markdown, by default, it is the syntax definition provided in the Markdown package, which defines the scope as text.html.markdown
